Spend less time in the waiting room when you fill out your forms before your appointment.

Patient Information

You should update this every year, or when your information changes.

Arrive 15 minutes before your appointment time.

Come early to update any necessary information. If you arrive more than 10 minutes after your appointment time, we may ask you to reschedule.

Let us know as soon as possible if you have to cancel.

Please do your best to cancel at least 24 hours before your appointment. If you cancel three times without advance notice, we may dismiss you from our practice.

Payment will be due at the time of your appointment.

Payment is due the day of service, including copayments, coinsurance and any other charges. If your care isn't covered by insurance, you are entitled to a 25% discount.

I'm a new patient.

For less time in the waiting room, fill out your new patient forms before your appointment.

Get the forms.

I need to refill a prescription.

Call and leave a message with your name, birth date, prescription and strength, pharmacy name and number and your doctor's name. Allow 24 to 48 hours for your request. We won't contact you about the request unless there is a problem.

We cannot refill some prescriptions.

You may be required to see your provider before we can refill your medication. We also do not accept refill requests from local or mail order pharmacies — you must make the request for these refills.

I'm waiting on test results.

You'll receive your test results in 7 to 10 days by mail or telephone.

I'm waiting on a referral.

Allow up to 7 days for your referral to go through. Once it does, our office or the specialist's office will call you with appointment information. If you make your own appointment, please let us know as soon as possible so we can obtain any prior authorizations that may be required.

I need to file insurance/disability forms.

Please bring your insurance cards with you to your appointment. We will file up to two insurance claims for no charge. 

I am a member of an HMO (Health Maintenance Organization).

Please bring a written referral form from your primary care physician to all visits. If we do not have this form, we will have to reschedule your appointment We try to help with these forms when we can, but many doctors cannot provide this form the same day.
